Financial Disclosure Requirements D.R.M.C. Chapter 2, Article V
All elected and appointed officers are required to submit an officer financial disclosure statement annually.
Pursuant to D.R.M.C. 2-72(e) all elected and appointed officers are required to disclose certain financial interests of the officer, or his or her immediate family members.
This statement covers the reporting period of January 1 - December 31, 2019.

Disclosure with Respect to Family Members
I or my spouse/domestic partner own at least 1% of a business or an interest in a business valued at over $5,000. This category includes stocks, LLC or LLP memberships, or other similar ownership interests.
Disclosure with Respect to Income
I received income in excess of $2,000 from any source other than the City and County of Denver, including capital gains, whether or not they are taxable. This could include interest, dividends, or other investment income. You do not need to report income from the City.
Disclosure with Respect to Lobbying Employment
I was compensated for lobbying for a person or entity, including an employer, other than the City.
Disclosure with Respect to Private Trust
I benefit from a private trust with assets valued at $5,000 or more.
Disclosure with Respect to Real Property
I own property or interest in a property, or had an option to purchase a property in Colorado during 2019. This includes rental properties and properties co-owned with another person, and properties held through a corporation or partnership. This does not include your personal home. If you only own your personal home and have no other interests in real property, please select “No”.
Disclosure with Respect to Transfer of Assets
I transferred assets of $5,000 or more to an immediate family member in the last two years. This can include gifts of cash, items, or real property.
Disclosure with Respect to Creditors
During 2019, I owed one or more creditor(s) an amount in excess of $5,000. This can include home and car loans, and credit card balances over $5,000 carried from month-to-month.
Disclosure with Respect to Employer
List the name of any current employer and any employer for whom the officer worked during the period from January 1 through December 31 of 2019.
